Attention TJ Regional Health Employees!
Step 1: Complete the Personal Health Survey. Once your "Date of Birth" and "SSN (last4)" are verified, you can begin the survey. The survey can be completed on most computers, tablets and
mobile devices with internet access and will take 15 minutes. The Personal Health Survey must be completed before your Biometric Screening appointment. Once you have reached the end of the survey,
click Submit to complete.
Steps 2 & 3: Register for Biometric Screening and complete a Tobacco and Nicotine Affidavit. Once you complete and submit the Personal Health Survey, there will be a link that states "Login to Your Personal Health Portal." Follow the prompts to create a username and password. You can register for both Biometric Screening and complete the Tobacco and Nicotine Affidavit under the Engagement tab in the portal.

Please login below using your DOB and the
last four of your SSN. Once your credentials have been verified, you will then be directed to your Personal Health Survey. Once you've completed that, you will want to schedule your
Biometric Screening appointment at one of the locations listed in the Health Portal.
